Job summary

Job Path in New York is hiring Direct Support Professionals to join the Community Connections program across Brooklyn, Queens, Manhattan and the Bronx. You will provide one-on-one supports, helping people access their communities and develop meaningful relationships.

Schedule includes Saturdays and Sundays with 5–6 hour shifts, plus possible weekday hours. You will work in a dynamic team, document progress, and support individuals to reach personal goals with dignity and respect.

Qualifications

• High school diploma or GED required.
• Prior experience is a plus, but not essential.
• Active, people-oriented work with sensitivity to disabilities.

Responsibilities

• Provide individualized supports to connect people to their communities.
• Develop connections with family and community members.
• Help individuals participate in volunteer jobs, classes, and social events.
• Support independent living skills and personal/professional goals.
• Document case notes, monthly summaries, and timesheets on schedule.
