Job summary

InterCommunity, Inc. is hiring a Community Support Specialist to deliver skill-building, engagement, and care coordination within the Community Support Team. Services are community-based, including in clients’ homes and various settings across our service area.

The role supports recovery through collaborations with clinicians, primary care, and community resources. A bachelor’s or associate degree in health or behavioral health or Recovery Support, plus relevant experience, is required.

Qualifications

• Bachelor’s or associate degree in health or behavioral health or Recovery Support Specialist.
• Equivalent experience in a community health center or behavioral health setting may be considered.
• Experience serving individuals in recovery from mental illness and substance use/addiction.

Responsibilities

• Collaborate with the clinical team to identify required services based on client needs.
• Support client recovery by joining OP clinicians, Primary Care, and participating in sessions as needed.
• Provide linkages with community resources and complete required treatment documentation.
• Engage clients and support attendance by completing tracking documents and reports.
• Assist during crisis assessments and determine need for detox and admission to detox facility.
• Provide education and support to families and support networks as needed.
• Transport clients as required with a reliable and insured vehicle.
