Job summary

InterCommunity, Inc. is a Federally Qualified Health Center look-alike providing accessible care. This part-time role supports addiction services, primary care integration, and behavioral health across community health centers and partner programs.

The position requires a Peer Support Specialist Certification or higher in Human Services, CPR within 60 days, and a valid driver license. You will supervise, coordinate care, and assist with transportation and documentation.

Qualifications

• Peer Support Specialist Certification or higher in Human Services required.
• CPR certification within 60 days of start date is required.
• Valid Driver License required.

Responsibilities

• Provide milieu supervision and coordinate social-rehabilitation groups and programs.
• Conduct care coordination and manage a case load of clients.
• Ensure safety of clients and the facility; perform safety checks.
• Maintain confidential progress notes and related documentation.
• Provide transportation as needed and complete agency errands.
• Occasionally work beyond scheduled hours to cover shifts.
• Attend mandatory meetings and trainings; time is paid work.
• Support program participants with dignity and respect.

Education
