The Peer Support Specialist helps engage clients in treatment, coordinates care, and links them with community resources across multiple centers in East Hartford, Hartford and South Windsor. Qualifications

• Recovery Support Specialist training through Advocacy Unlimited is required.
• Equivalent experience in a community health center or behavioral health setting may be considered.
• Experience working with individuals in recovery from mental illness and substance use/addiction.

Responsibilities

• Engage clients and support ongoing attendance by completing tracking documents and reports.
• Provide linkages to clinical, medical, social, educational and rehabilitative services.
• Collaborate with the clinical team to identify required services and plan aftercare.
• Assist in crisis assessment and facilitate admission to detox as needed.
• Perform related duties including transportation of clients with a reliable vehicle.
