Cumberland County Guidance Center in Millville, NJ is seeking a Full-Time Community Support Services Case Manager to help individuals and families achieve wellness through housing support, community integration, and access to vital services.

The role requires a mental health-related bachelor's or RN/LPN with relevant experience, is onsite, and offers a competitive salary and benefits package including medical, dental, vision, 401(k), life insurance and paid time off.

Qualifications

• Bachelor’s degree in a mental health‑related field or RN/LPN with relevant experience.
• Proficiency with community resources to connect clients to needed services.

• Valid New Jersey driver’s license with a clean five‑year driving record.
• Willingness to work a flexible 40‑hour schedule including evenings, weekends, holidays, and on‑call rotation.

Your day to day as a Community Support Services Case Manager

As a Case Manager for the Community Support Services Program at Cumberland County Guidance Center, you will be at the forefront of advocacy and support for individuals with mental illness living in the community. You will help clients secure housing, maintain their integration into the community, foster their self‑sufficiency and independence, promote the use of vital community services, support clients in travel and fiscal management, and facilitate access to healthcare. You will collaborate with treatment providers to develop tailored Individual Rehabilitation Plans, documenting progress to ensure effective service delivery. By assisting individuals in honing their social skills and daily living capabilities, you will empower them to thrive in their environment.

What we're looking for
