Job summary

Center for Family Services in Cumberland County, NJ, is seeking a full-time Case Manager to assess needs, develop treatment plans, and connect clients to essential resources for recovery.

You will collaborate with Mental Health Peer Recovery Specialists and program staff to ensure comprehensive support and advocate for clients to access necessary services and care.

Qualifications

  • Bachelor's or master's degree in psychology, counseling, social work or related behavioral health field.
  • Three years' experience in Behavioral Health services.
  • Knowledge of formal/informal community systems and resources.
  • Experience to perform competently case management activities.
  • Three years' experience with high-need individuals with mental health disorders.

Responsibilities

  • Review, complete, maintain and monitor documentation within timeframes per regulations.
  • Complete client intakes and assessments covering transportation, health care, etc.
  • Assess MAT/MOUD needs and document barriers to recovery.
  • Collaborate with clients to set goals and link to resources.
  • Connect clients to community resources for housing, health, legal needs.
  • Access and facilitate behavioral health services when needed.
  • Document services, utilization and outcomes in Care Logic, DocuSign, etc.
  • Collaborate with Mental Health Peer Recovery Specialists on referrals.
  • Collaborate with Supervisors on recruitment and HR activities.
  • Conduct thorough case reviews per program policies.
  • Assist leadership with data gathering and daily/monthly/yearly reports.
  • Support staff development within agency guidelines.
  • Deliver services in a culturally competent and linguistically appropriate manner.

Program Summary

The Case Manager is responsible for assessing client needs, developing individualized treatment plans, and connecting individuals to essential resources for recovery. This position requires collaboration with Mental Health Peer Recovery Specialists, program supervisors, and community partners to ensure a comprehensive support system for clients. The Case Manager plays a crucial role in advocating for clients and ensuring they receive the necessary support for successful treatment and sustained recovery.

Duties and Responsibilities
  • Review, complete, maintain, and monitor all required documentation within established timeframes in accordance with Federal, State, County, accreditation, and agency regulations, policies, and procedures.
  • Complete comprehensive client intakes and case management assessments addressing life domains such as transportation, legal services, health care, and family strengths/needs.
  • Assess client MAT/MOUD needs and identify barriers to recovery, ensuring all relevant information is documented accurately.
  • Collaborate with clients to establish goals and link them to appropriate resources based on case management assessments.
  • Connect clients with community resources to address specialized needs such as: mental health support, chronic/acute health care, housing, or legal or criminal justice-related assistance.
  • Access and Facilitate access to behavioral health when necessary.
  • Monitor and ensure accurate documentation of services, utilization, and outcomes in systems such as Care Logic, DocuSign, and/or any other electronic technology systems.
  • Collaborate with Mental Health Peer Recovery Specialists on linking, advocating, and referring clients to appropriate services.
  • Collaborate with Program Supervisors on recruitment, retention, and HR-related activities.
  • Complete thorough and comprehensive case reviews per program policies and procedures.
  • Support program leadership in recording, gathering, researching, and documenting program data for daily, monthly, and yearly reports as required.
  • Support program leadership in evaluating, training, promoting, and guiding staff within approved agency guidelines.
  • Deliver services in a culturally competent and linguistically appropriate manner, ensuring alignment with program and client needs.
Requirements
  • Bachelor's or master's degree in psychology, counseling, social work, or other behavioral health profession.
  • Three years' experience in Behavioral Health services
  • Shall possess knowledge in formal/informal community systems and resources.
  • Must possess the knowledge, skills and experience necessary to perform competently case management activities.
  • Must have at least three (3) years' experience working with high need individuals involved with mental health disorders.
  • Shall demonstrate evidence of working with behavioral health populations or evidence of coursework.
  • Requires car, valid registration, insurance and driver's license and must maintain compliance with CFS Motor Vehicle policies
