Clarity Care, Inc. is hiring Community Support Providers to help Members live independently at home, work, and in the community. You’ll coach daily living skills, support personal goals, and promote meaningful connections while receiving paid training with no certifications required.

We offer flexible shifts in Green Bay, Oshkosh, and surrounding areas. You’ll empower Members through hands-on support, crisis prevention, and collaboration with families and other professionals to ensure quality

Responsibilities

• Empower Members to develop everyday living skills (cooking, housekeeping, budgeting).

• Support community integration, recreational activities and employment opportunities.
• Assist with personal care and daily living activities per care plans.
• Monitor medications and document according to procedures.
• Collaborate with families and other professionals for coordinated care.
