Job summary

Clarity Care Inc. seeks compassionate Community Support Providers (CSPs) in Green Bay, Oshkosh and Menasha, WI. You’ll help Members live independently, coaching daily living skills and supporting them at home, work, and in the community.

No certifications are required initially; paid training is provided and shifts are flexible to meet scheduling needs. Starting pay is $16.50 per hour with opportunities for growth.

Qualifications

• High school diploma or equivalent education.
• Valid Wisconsin driver’s license and proof of auto insurance.
• Ability to complete caregiver background checks.
• Ability to respond to challenging situations and use crisis prevention techniques.

Responsibilities

• Help Members develop everyday living skills including cooking, housekeeping, laundry, shopping and budgeting.
• Encourage problem-solving and independence through coaching and hands-on support.
• Support Members in setting and achieving personal goals and celebrate progress.
• Provide job coaching and workplace support to help Members develop skills and employment opportunities.
• Collaborate with families and other professionals to ensure coordinated, quality care.

• Assist with personal care and daily living activities based on each Member’s individual needs and preferences.


• Support medication routines through monitoring and documentation in accordance with care plans and agency procedures.


• Promote a safe, respectful environment that protects Member rights, dignity and independence.


• Collaborate with families, employers and other support professionals to ensure coordinated, quality care.

Basic Requirements

• At least 18 years of age.


• High school diploma, equivalent education and/or relevant work or life experience.


• Valid Wisconsin driver’s license, proof of auto insurance and ability to meet agency driving standards.


• Ability to successfully complete required caregiver background checks.


• Ability to respond appropriately to challenging situations and use crisis prevention and de‑escalation techniques as needed.

Preferred Qualifications

Experience supporting individuals with physical or developmental disabilities or mental health needs, including severe and persistent mental illness.
