Job summary
Heritage seeks a Program Manager, Community Support, to lead a multidisciplinary team delivering intensive community-based rehabilitation and outreach services for adults with mental health and co-occurring disorders. You will supervise clinicians, monitor quality measures, and ensure services align with treatment plans across on-site and off-site locations.
The role requires a Master’s degree in a related field and licensure or strong eligibility.
Qualifications
- Master’s degree in counseling, social work, psychology, or related field.
- Licensure as CADC/LSW/LCSW/LPC/LCPC is highly recommended.
- Experience with mental health or substance use diagnoses preferred.
Responsibilities
- Provide team and individual supervision.
- Monitor quality measures and standards.
- Deliver direct services off-site when appropriate per service plan.
- Complete assessments and required documentation.
- Develop crisis and treatment plans with the team and clients.
- Coordinate referrals and ensure access to services.
- Communicate effectively with staff and families.
- Support progress toward rehab, resiliency, and recovery goals.
