Job summary

Heritage is seeking a Program Manager to lead a Community Support Services team providing recovery-oriented, community-based rehabilitation and outreach for adults with mental health and co-occurring disorders.

The role supervises the team clinically and administratively and functions as a practicing clinician. Services occur across Heritage locations, both on-site and off-site, including clients' homes and community settings.

Qualifications

  • Master's degree in counseling, social work, psychology, or related field.
  • Licensure as CADC/LSW/LCSW/LPC/LCPC highly recommended.
  • Experience with mental health or substance use diagnoses preferred.

Responsibilities

  • Provide team and individual supervision.
  • Complete supervisor tracking to monitor quality measures and standards.
  • Provide direct services to individuals off-site 60% of the time.
  • Complete assessments including IATP and LOCUS, with required documentation.
  • Develop crisis and treatment plans in collaboration with individuals and team.
  • Collaborate and communicate with staff to ensure coordinated care.
  • Provide services to reduce crisis episodes and improve community functioning.
  • Participate in bi-weekly team meetings and maintain knowledge of each individual.
  • Maintain open communication with agency staff, family, and others.
  • Work cooperatively to ensure access to services identified during treatment planning.
  • Ensure clinical activities comply with organizational standards.
  • Assist in referrals and linkages to programs.

Job description

This leadership position will serve as the lead for a Community Support Services team. The team provides recovery- and resiliency-oriented, intensive, community-based rehabilitation and outreach services for adults with mental health and co-occurring disorders. The program utilizes a team-based approach consisting of mental health rehabilitation interventions and supports necessary to assist individuals in achieving and maintaining rehabilitative, resiliency, and recovery goals.

The Program Manager serves as the clinical and administrative supervisor of the team while also functioning as a practicing clinician. Services are provided across a variety of Heritage locations, both on-site and off-site, including, but not limited to, the individual’s home, community settings, healthcare locations, and other social service agencies.

Core Responsibilities:
  • Provide team and individual supervision.
  • Complete supervisor tracking to monitor quality measures and standards.
  • Provide direct services to individuals assigned to the team, with at least 60% of services provided off-site during times and in locations most convenient for the individual served, in accordance with the individual’s service plan.
  • Complete all necessary assessments, including the IATP and LOCUS, as well as required documentation and billing notes, in accordance with Rule 132/140 standards.
  • Develop and implement crisis plans and treatment plans outlining treatment goals and interventions in collaboration with the individual served, the team, other providers, and other parties identified by the individual served, following the standards of Rule 132/140, payers, and accreditation bodies.
  • Collaborate and effectively communicate with staff to ensure coordinated care and provide referrals for individuals who no longer require this level of care.
  • Provide direct services that promote the reduction of crisis episodes and increased community functioning in order to support rehabilitative, resiliency, and recovery goals.
  • Participate in bi-weekly team meetings and maintain current knowledge and understanding of the treatment, progress, goals, and needs of each individual served by the team.
  • Maintain open and effective communication with agency and provider staff, family members, and others regarding identified needs, service needs, treatment plan follow-up, and adaptations needed to enhance progress toward goals.
  • Work cooperatively with staff to ensure appropriate access to services as identified during the treatment planning process.
  • Ensure that all clinical activities performed by the team comply with organizational standards, policies, and procedures.
  • Assist in ensuring that referrals and linkages to programs are addressed in a timely manner.
Knowledge, Skills, and Abilities:
  • Ability to engage individuals and decrease barriers to accessing treatment.
  • Knowledge of behavioral health diagnoses and use of the DSM-5.
  • Knowledge of co-occurring diagnoses and evidence-based treatment approaches.
  • Knowledge of case management techniques and community resources, including psychiatric, substance use, medical, and other community services.
  • Effective oral and written communication skills.
  • Ability to work effectively in a team-based environment.
  • Strong customer service skills.
  • Flexibility and adaptability.
  • Ability to navigate an electronic health record.
  • Proficiency in Microsoft Word and Excel.
Education and Experience:

The successful candidate will possess a Master's degree in counseling, social work, psychology, or a related field. Licensure as a CADC, LSW, LCSW, LPC, or LCPC is highly recommended. Ideal candidates will have previous experience working with individuals with mental health or substance use diagnoses.

A valid driver's license, reliable means of transportation, and proof of current automobile insurance are required for this position.

Salary: $65,000 to $85,000
