Job summary

Adjoin is seeking a Direct Support Coordinator in California to empower adults with disabilities through direct services and training, ensuring safety, independence, and quality of life.

The role supports case management, coordinates training, and helps maintain CSPs while coordinating with supervisors, regional centers, and families. Travel up to 75% is required; a valid CA driver’s license is mandatory and you must pass background checks.

Qualifications

• 18+ years old and high school diploma or equivalent.
• Bachelor’s degree or 3 years of relevant experience working with individuals with disabilities.
• Must have valid CA driver’s license and insured vehicle.
• Pre-employment drug screen and background check are required.

Responsibilities

• Advance client decision-making and participation in daily activities.
• Promote independence while assisting with meals, grooming, and household tasks.
• Implement individual training and support programs as directed.
• Maintain communications with supervisor, clients, families, and partners.
• Complete CSPs, reports, evaluations, and assessments as required.
• Provide direct support at least 60% of logged time to ensure CSP adherence.
• Transport clients to appointments and community activities safely.
