Job summary

Phaxis is seeking a compassionate Social Worker for the Community Guardian Program in New York City, serving adults across the five boroughs. You will provide case management, advocacy, referrals, and community-based support to clients in NYC.

Qualifications

• Master’s degree in Social Work or related field.
• Strong writing and documentation skills.

• Ability to travel across NYC five boroughs.

Responsibilities

• Manage and monitor an assigned client caseload.
• Assess client needs, safety, functioning, and eligibility for services.
• Advocate for clients and develop individualized service plans.
• Coordinate referrals for Medicaid, Medicare, SNAP, home health aides, public assistance, legal aid, and more.
• Conduct home, facility, and community visits throughout NYC.
• Attend court hearings and community meetings.
• Maintain case records and documentation using agency databases.
• Collaborate with community agencies, healthcare providers, legal teams, and other professionals.
