Job summary

NYC Department of Social Services is seeking an APS Social Worker to provide psychosocial services across seven borough offices. You will consult on complex cases involving abuse and neglect, contribute to multidisciplinary teams, and support discharge planning with hospitals, clinics, and nursing facilities.

Qualifications

• Master's Degree in Social Work from an accredited school of social work.
• Licensure as LMSW or LCSW within one year of appointment.
• Residency requirement for NYC may apply.

Responsibilities

• Provide psychosocial services to individual clients, families, or groups of adults/children.
• Conduct psychosocial assessments for supportive housing applications and other services.
• Link clients with public and private programs for needed services.
• Collaborate with hospitals, clinics, nursing homes and other facilities for discharge planning.
• Assist in preparation of treatment plans and legal documents for Article 81 court orders.
• Interview and assess clients to determine community functioning.
• Evaluate the quality and appropriateness of service referrals.
