Job summary

Phaxis in Brooklyn, NY is seeking an experienced Case Manager to serve older adults and families in a community-based social services program.

You will assess clients' social and emotional needs, develop individualized case plans, connect clients with community resources, provide advocacy, and coordinate services to help maintain independence and improve quality of life.

This full-time, long-term temporary role prefers bilingual Russian speakers and requires a Bachelor’s Degree in Social Work.

Qualifications

• Bachelor’s Degree in Social Work required.
• Strong case management, documentation, and communication skills.
• Comfortable working directly with older adults and their families.
• Computer proficient and able to learn program-specific software.
• Ability to travel to clients' homes using public transportation.
• Compassionate, organized, dependable, and able to work independently in the community.

Responsibilities

• Assess the social and emotional needs of older adult clients and their families.
• Develop and implement individualized case plans.
• Provide client and family counseling, guidance, and support.
• Screen clients for benefits eligibility and assist with entitlement applications.
• Advocate for clients and connect them with appropriate community resources.
• Coordinate services including transportation, home care, Meals on Wheels, Medicare, Medicaid, housing, legal assistance, and other supportive services.
• Evaluate clients' ability to manage their affairs and identify potential abuse or exploitation concerns.
• Make appropriate referrals to Adult Protective Services and other community agencies.
• Conduct ongoing reassessments and update case plans based on changing client needs.
• Maintain accurate case records, reports, and required documentation.
• Participate in community meetings, education groups, and agency trainings.
• Travel to clients' homes as required.
