Gateways Hospital and Mental Health Center, located in Los Angeles, is a non-profit organization providing comprehensive mental health services for Children, Adolescents, Transitional Aged Youth, and Adults. The organization includes a 55-bed hospital and six outpatient facilities, employing approximately 400 staff members.

Position Summary

The Community Services Manager is a key member of the management team overseeing Home Visit Counselors and Case Managers to ensure contractual compliance with the Department of State Hospitals. The role involves managing home visits, urinalyses, case management services, and acting as a liaison to residential facilities and community resources.

Essential Duties
  1. Supervise and evaluate staff, including recruiting, training, coaching, and disciplinary actions.
  2. Ensure comprehensive and compliant home visits, maintaining schedules and documentation.
  3. Coordinate with residential facilities, ensuring proper communication and collaboration.
  4. Identify and facilitate residential placements, ensuring proper documentation and follow-up.
  5. Address residential concerns proactively and engage with community partners.
  6. Explore and link additional community residential resources.
  7. Stay informed on trends in community-based residential services.
  8. Ensure timely linkage of clients to community resources and proper documentation.
  9. Oversee urinalysis practices, schedules, and documentation, liaising with labs as needed.
  10. Monitor urinalysis results and communicate findings promptly.
  11. Ensure vehicle safety standards, maintenance, and documentation.
  12. Conduct quality assurance reviews of electronic health records and logs.
  13. Provide timely updates to treatment team members.
  14. Manage staff attendance, timecards, and performance reviews.
