Job summary

Optimal Case Management in Salisbury, MD is seeking a Case Manager / Coordinator of Community Services (CCS). In this full-time role, you will build trusting, person-centered relationships with individuals and their families and coordinate services around their needs and goals.

The position requires regular in-person community work, travel across the service region, and meticulous documentation to meet regulatory requirements.

Qualifications

• Possess a Bachelor's or graduate degree in social work or another health or human services field.
• Complete a supervised field experience in case management, behavioral health, or vocational rehabilitation or two years of direct clinical care.

Responsibilities

• Build trusting, person-centered relationships with individuals and their families or support systems.
• Learn what matters most to each person and translate goals into meaningful supports and services.
• Coordinate and monitor services across providers, agencies, and community resources.
• Connect individuals and families with community, healthcare, employment, housing, education, financial, and social resources.
• Monitor services to meet identified needs and advocate when barriers arise.
• Participate in planning meetings and collaborate with multiple stakeholders.
• Maintain accurate, timely, and person-centered documentation of case management activities.
• Manage an assigned caseload while meeting quality, productivity, and compliance expectations.
• Participate in supervision, training, and professional development.
• Maintain confidentiality and protect sensitive information.

Education

Bachelor's or graduate degree in social work or another health or human services field

Two years of direct clinical care experience or supervised field experience
