NLACRC is seeking a Consumer Services Coordinator for the SFV area to provide information, advocacy, and service coordination for individuals and families in need. The role emphasizes coordinating services, supporting plan development, and ensuring documentation compliance.

You will engage in interdisciplinary meetings, advocate for clients, and may cover on-call shifts. A bachelor’s degree in a human services field and relevant experience are required.

Competenze

• Bachelor’s degree in psychology, social work, sociology, or related human services.
• At least two years of related experience; master’s may substitute.
• Knowledge of family systems and person-centered planning.
• Excellent verbal and written communication; proficient with MS Word and MS Outlook.

Mansioni

• Provide information, advocacy, and service arrangement for consumers and families.
• Develop and monitor program plans for individuals.
• Identify and coordinate services for individuals and families.
• Document forms and reports per regulations and NLACRC policies.
• Advocate for individuals served by community agencies.
• Attend interdisciplinary meetings with individuals, families, providers, advocates.
• Rotate officer-of-the-day duties and provide on-call coverage.
