APLA Health Center in Los Angeles, CA seeks a CSS Case Worker to deliver community support and case management for general patients across CDU/MLK sites. You will assess SDOH needs, coordinate health, social, and community services, and maintain thorough records in the EHR.

Working with medical, behavioral health, and admin staff, you will build community connections, conduct referrals, and support patients in navigating chronic conditions, housing, food access, and supportive programs.

Formación

• BA/BS in public health, social work, health education, nursing, education, or related field with 1 year case management experience.
• Or high school diploma with 4 years of relevant case management experience.
• Experience with diverse, medically underserved populations including LGBTQ+ individuals and those affected by trauma.

Responsabilidades

• Assess SDOH needs and provide education on health, social, and community resources.
• Maintain a caseload of 60-80 patients with intake, case management and follow-up.
• Record detailed patient interactions and progress in the EHR and CSS tools.
• Collaborate with medical, behavioral health, and admin staff within clinics.
• Engage external stakeholders, coordinate referrals and warm handoffs.
• Identify community assets to support patient needs and outcomes.
