Aufgaben

• Follows up to ensure appropriateness of referrals.
• Supports the integrated care process.
• Serves as one of the communication links between medical and behavioral health providers.
• Addresses significant social service needs such as food insecurity, housing, special needs services, substance use, transportation, educational and vocational issues.
• Arranges, coordinates, monitors, evaluates and advocates for a package of services to meet the specific client's complex needs including assigned case load of ECM program participants.
• Other duties needed to help support the organization's mission, vision, and values in accessing higher quality of health and wellness services.
