Job summary

Colorado Coalition for the Homeless seeks a Nurse Case Manager to deliver community-based nursing care within the bounds of nursing ethics and state/federal laws. The role focuses on patients living on the streets and in supportive housing, providing triage, education, and coordinated care in collaboration with an integrated health team.

Responsibilities include medication administration, care planning, and ongoing professional development within a trauma-informed, harm-reduction model.

Qualifications

• Associates, Bachelors or Masters in Nursing from an accredited college or university.
• Current Registered Nurse licensure in the State of Colorado.
• Basic Life Support certification required.

Responsibilities

• Provide skilled nursing care in community settings per standards and protocols.
• Conduct triage assessments and provide appropriate care recommendations.
• Educate patients on chronic and acute health conditions and prevention.
• Administer medications safely per protocols.
• Collaborate with a multidisciplinary health team to deliver care.
• Coordinate care with internal programs and health centers.
