Job summary

Commonwealth Care Alliance is seeking an Integrated Care Clinical Manager (RN) to deliver high-quality, home- and community-based care coordination for dually eligible enrollees with complex medical and social needs. You will lead an interdisciplinary team, develop individualized care plans, and ensure smooth communication among providers, families, and community resources.

You will educate members about benefits, support preventive care, and navigate Medicare/Medicaid services while traveling

Qualifications

• Associates of Science in nursing from an accredited program required.
• Registered Nurse with the ability to independently serve people with complex medical, behavioral, and social needs.
• Prior care coordination or case management experience preferred.
• Medicaid/Medicare managed care experience preferred.
• Clinical Field/Community Based Training a Plus.

Responsibilities

• Engage enrollees in homes and community settings to build care management relationships.
• Coordinate with providers and resources to ensure seamless transitions.
• Perform comprehensive assessments and risk evaluations.
• Lead the interdisciplinary care team to create holistic care plans.
• Oversee utilization of long-term services and supports to keep enrollees in their preferred location.
• Assist enrollees in accessing community resources and Beneficiary education.
