Job summary

AbsoluteCare is seeking a Resource Manager to lead CBO partnership development, oversee budget and resource management, and supervise a team of SDOH Coordinators. The role focuses on reducing social barriers to health and driving KPI achievement across programs.

You will oversee a comprehensive Community Resource Guide, collaborate with community partners, and ensure housing policy knowledge informs member pathways. Strong leadership and data-driven decision making are essential.

Qualifications

• 3+ years of management experience leading teams across functions.
• Bachelor’s degree required; MSW or public health preferred.
• 2+ years experience in community agencies or healthcare.
• Experience achieving KPIs.
• Experience with EMR and clinical management platforms.
• Strong IT skills: Word, PowerPoint, Excel.
• Organizational skills and ability to multi-task.
• Driver’s license and reliable transportation.

Responsibilities

• Lead SDOH team in a matrixed, cross-functional organization.
• Provide critical reasoning, decision-making, and problem-solving to analyze situations, assess risks, and implement preventive actions.
• Own performance management through tracking KPIs, quality metrics, coaching and corrective actions.
• Leverage data, KPIs, and reporting to track performance and address gaps.
• Foster partnerships with community-based organizations supporting SDOH gaps.
• Develop relationships with community partners to maintain a vetted network for member needs.
• Manage and maintain a robust Community Resource Guide to connect members to resources.
• Serve as housing SME with knowledge of housing policy and opportunities.
• Collaborate with shared services to implement processes and maintain consistency across teams.
• Lead weekly team meetings to align priorities and goals.
• Meet with Engagement Director bi-weekly on progress and barriers.
• Ensure proper documentation and reporting in technology systems.
