Job summary

AbsoluteCARE Medical Center & Pharmacy is seeking an SDOH Coordinator to assist care teams in resolving social determinants of health needs for our members. You will link patients to community resources, assist with applications, and follow up to ensure access to benefits.

The role requires knowledge of housing, SNAP, transportation, health plan benefits, and related programs, plus strong communication and organizational skills for timely follow-up.

Qualifications

• Bachelor's degree or equivalent experience serving complex populations.
• 2 years' experience with Medicaid/Medicare or uninsured populations.
• Extensive knowledge of local community resources and entitlement processes.
• Strong written and oral communication and teamwork skills.

Responsibilities

• Receive referrals to address members' SDOH needs and connect them to resources.
• Coordinate services with community resources (housing, benefits, food, utilities).
• Document interactions in electronic health records with timely updates.
• Collaborate with care teams and behavioral health for referrals and follow-up.
• Engage members in the community and within the center; track progress.
