Job summary

AbsoluteCare is seeking a dedicated SDOH Coordinator to assist primary care teams by connecting members with community resources, guiding them through benefit applications, and following up to ensure access.

You will maintain knowledge of programs, coordinate with care managers, and document interactions in the electronic health record, with CPR/BLS certification required and a valid driver's license for community visits.

Qualifications

• Bachelor's degree in public health, social work, or related health care field (or 3 years’ equivalent experience).
• At least 2 years experience serving complex populations (e.g., Medicaid, Medicare).
• Knowledge of local community resources and entitlement processes.
• Strong teamwork and communication skills.
• Ability to work independently and manage multiple tasks.
• Valid driver’s license and reliable transportation.
• CPR/BLS certification required.
• Experience with electronic health records preferred.

Responsibilities

• Receive referrals from the Integrated Care Team to address members’ SDOH needs.
• Assist with referrals and coordinating services with community resources (housing, benefits, food, utilities, transportation).
• Update the local Center’s Community Resource Guide.
• Collaborate with ICT for services not covered by the health plan.
• Work with the Resource Specialist on member fund requests for SDOH expenses.
• Coordinate with Behavioral Health for referrals.
• Participate in care team meetings and case consultations.
• Follow PCP action plans to meet member needs.
• Document all interactions in the electronic health record.
• Prioritize tasks to meet response time metrics.
