Community Resource Navigator

Location: Pittsburgh, PA
Job Type: Full-Time (40.00 hours per week)
Salary: $24.52 per hour ($51,000)

At Human Services Administration Organization (HSAO), we are dedicated to providing comprehensive service coordination to individuals and families across Allegheny County. Our mission spans behavioral health, drug and alcohol services, justice‑related programs, and community support. FUNCTIONAL DEFINITION

Focused on people with complex needs, the primary responsibility of the Community Resource Navigator will be to dedicate time as an intermediary between people that need in depth navigation and assistance & human services. This Community Resource Navigator will act as a link by ensuring connections are made with community agencies and DHS systems. The work is complex, requiring analytical thinking, a high degree of initiative, and the exercise of independent judgment.

DUTIES AND RESPONSIBILITIES
  • Make connections with formal DHS service providers as a team approach to the next steps if person, child or family is already receiving services.
  • Attend and/or coordinate teaming meetings as needed.
  • Triage and make appropriate referrals to contracted agencies and community partner agencies.
  • Follow up with individuals/families and the agencies working with them to ensure that appropriate connections are made.
  • Lead the coordination of a network of navigation teams within DHS to ensure that all have the following:
    • common training
    • cross training
    • a mechanism to collect and share all resources/referrals
    • intent to support each other in their role as a ‘connector’
SKILLS AND KNOWLEDGE
  • Excellent working knowledge of the educational, human services and court systems in Allegheny County
  • Strong written and oral communication skills, including public speaking and formal training
  • Approachable, flexible organized and ready to trouble shoot/problem solve at a moment’s notice
  • Assessment and service coordination skills necessary to develop and maintain a positive relationship with all partners, focusing on strengths based/family focused strategies.
  • Community building, outreach and networking skills
  • Strong organizational skills and ability to work independently as supervision will be off‑site
  • Willingness to research services and find answers to difficult/complex questions
  • Proficient in Internet “searching” skills, and Microsoft Windows programs including Word, Excel PowerPoint, Access, Adobe Acrobat and Microsoft Publisher
ESSENTIAL REQUIREMENTS
  • Proficient with computers specifically Microsoft Windows programs including Word, Excel, PowerPoint, Access, Adobe Acrobat, and Microsoft Publisher.
  • Must possess ACT 33/ 34 and FBI Clearances.
  • Must possess any and all background checks required by DHS and Pittsburgh Public Schools.
  • Have a valid Pennsylvania motor vehicle license and access to a personal vehicle.
  • Ability to work independently and multi-task
  • Applicants should be inherently curious and be able to community effectively with a diverse group of stakeholders (people in need, human service providers, DHS staff, court partners).
  • Full-time position requiring 38.75 hours per week.
EDUCATION AND EXPERIENCE

Bachelor’s Degree in social services, education, or related field plus a minimum of one year of experience working in social services, education or related fields.
