Job summary

Northwest Human Services is seeking a Community Outreach & Engagement Coordinator to liaise with patients, build trust, and connect people to essential healthcare and social resources in the Salem area.

You will engage patients, work with care teams, and plan community outreach events while collaborating with partners to expand access to services. Qualifications include a relevant degree and BLS certification, with an Oregon driver’s license.

Qualifications

• What You'll Bring: At least one year of experience in a human service-related or healthcare environment.
• Associate or bachelor's degree in public health, behavioral science, social services, or a related field.
• Current Basic Life Support (BLS) for Healthcare Providers certification.
• Valid Oregon driver’s license and driving record acceptable for agency auto liability policy.

Responsibilities

• Build Trust & Engage Patients: conduct outreach to patients to encourage engagement with NWHS services and care teams.
• Provide education about available healthcare, social services, and community resources; use culturally responsive approaches.
• Connect patients with internal programs and external community organizations based on needs and eligibility.
