Job summary

Gwinnett County Government seeks a Community Relationship Development Specialist to coordinate health, human, and social service partnerships. The role focuses on navigating resident needs, including food insecurity, housing, utilities, and other social services, within Community Resource Centers.

Responsibilities include engaging partners, documenting services, providing outreach, and supporting warming stations and encampment efforts for vulnerable populations.

Qualifications

• Bachelor’s degree in social work or related field is required.
• CPR and First Aid certifications are required.
• One year of experience in case management or an equivalent combination of education and experience is required.

Responsibilities

• Identifying and collaborating with community partners onsite and within the community at large through active communication, decision-making, and problem-solving
• Serving residents, community organizations, and stakeholders in the community onsite and through One Stop for Help programs including warming stations and vulnerable populations
• Coordinating services with relevant community resources and other service providers to ensure a holistic approach to resident need and outreach
• Maintaining detailed, accurate records, case notes, progress updates, and documentation of services provided
• Monitoring participant progress and following up on referrals to connect residents to appropriate services
• Providing and collaborating with key resources and assistance necessary through mobile outreach and community programs
• Communicating and advocating with internal and external partners to identify and resolve community needs
• Providing education and outreach to raise awareness and engagement
• Participating in a rotational on-call schedule to support program operations outside regular hours
• Upholding ethical standards, cultural competency, and professional boundaries in service delivery
• Have knowledge of case management principles; applicable laws and regulations; counseling principles; and available resources to clients
