Job summary

Appalachian Mountain Health invites a Community Resource Advocate to connect patients with Medicaid and other supports, coordinating services across behavioral health, dental care, and community programs. You will maintain patient records, scrub schedules, and build strong partnerships with local resources to improve access to care for underserved populations.

Qualifications

• Experience with community resources and public benefits.
• Strong interpersonal communication and community-building skills.
• Able to manage time, meet deadlines, and respond to requests promptly.
• Proficient with technology and documenting in EHRs.
• Able to work with diverse populations in a healthcare setting.

Responsibilities

• Coordinate services to connect patients with Medicaid and other benefits.
• Refer patients to behavioral health, dental, and supportive programs as needed.
• Identify patients needing additional supports before appointments.
• Maintain accurate documentation in the patient’s medical record.
• Keep the Community Resource database up-to-date and collaborate with community partners.
