Job summary

Saint Anthony Hospital is seeking a Community Resource Educator to help clients access health care, social services, and public benefits. The role includes education, outreach, and coordination within the Community Wellness Program in Chicago, IL.

The function emphasizes enrollment assistance, data entry in EMR, and collaboration with local organizations. A bachelor’s degree and at least one year of related experience are required.

Qualifications

• Bachelor's Degree plus a minimum of one year work experience related to the job description.
• Basic knowledge of public benefits.
• Experience with data entry.
• Familiarity with community resources in the hospital’s service area.
• Comfort and familiarity with Latino and/or African American cultures.
• Excellent interpersonal skills.
• Must be able to work some outreach events during evenings or weekends.

Responsibilities

• Determine Medicaid/Medicare eligibility and submit public insurance applications.
• Provide basic and targeted case management and referrals to health and social services.
• Assist clients in applying for public benefit programs (Medicare, Medicaid, SNAP).
• Represent the Community Wellness Program at public forums and build partnerships.
• Maintain relationships with benefit program offices and organizations.
• Plan and execute community-based public benefits education and outreach activities.
• Participate in supervision, staff meetings, and in-service.
