Job summary

RWJBarnabas Health is seeking a Community Health Educator to support community health strategy by coordinating, implementing, monitoring and evaluating programs, including the Better Health initiative.

The role involves collaboration with hospital leaders, clinical departments and community partners to address health disparities, develop educational programs, and maintain education records with monthly reports.

Qualifications

• Bachelor's degree in public health, education, health administration, health education, or related field.
• Experience working with community stakeholders from diverse backgrounds.
• Position requires some evenings and weekends.

Responsibilities

• Coordinate, implement, monitor and evaluate community health programs and initiatives.
• Collaborate with hospital leaders, departments and community partners to address health disparities.
• Develop goals, strategies and performance measures for CHNA, CHIP and SIP.
• Serve as liaison to community health coalitions and ensure alignment with CHNA/SIP.
• Develop educational programs and facilitate sessions with clinicians or speakers.
• Maintain records and provide monthly data reports on education and screening activities.
