Job summary

Acacia Network seeks a Community Residence Aide in Bronx, NY to provide residents with high-quality care in a therapeutic setting under supervision. The role focuses on daily living assistance, service coordination, and documentation to ensure safety and well-being.

Under supervision, you will work with health teams to support residents’ social, educational, and personal growth while upholding program policies and cultural competence.

Qualifications

• Associate Degree or higher required.
• Desire to work with people facing mental health challenges and help them improve.
• Three years of experience in Human Services or Behavioral Health preferred.
• Positive attitude with excellent customer service skills.
• Ability to work well within a team and contribute positively.
• Excellent documentation skills, ability to meet deadlines and follow protocol.
• Comfortable working in a culturally diverse environment.

Responsibilities

• Conduct intake assessment planning, service coordination, service provision, and discharge planning.
• Provide assertiveness and self-advocacy training to promote residents’ growth.
• Assist residents with developing daily living skills.
• Coordinate with primary health and clinical care teams for seamless care.
• Assist in record keeping, monitoring, storage and self-administration of medication.
• Manage incidents per the organization’s policy and guidelines.
• Support mental health projects to meet program goals and contractual requirements.
• Assist in developing social skills to reduce isolation and encourage educational opportunities.
• Support ongoing learning through trainings and workshops.
• Provide services addressing dressing, hygiene, meals, cleaning, budgeting, and environment upkeep.
• Ensure timely restorative services and accurate chart documentation.
