Volunteers of America of Massachusetts is hiring a Community Support Navigator to provide community-based case management and recovery support for individuals facing mental health and housing challenges. The role includes re-entry planning, coordination with correctional facilities, and post-release follow-up across multiple service domains.

The position is full-time and on-site in Massachusetts, with a comprehensive benefits package and opportunities to collaborate with community partners.

Qualifications

• Bachelor’s degree in a related human services field preferred.
• At least two years of social service experience.
• Experience with correctional facilities and re-entry preferred.
• Excellent communication, organizational, and computer skills with valid driver's license.

Responsibilities

• Manage a mixed caseload of 30–40 participants transitioning from correctional facilities.
• Provide case management, care coordination, and recovery support.
• Engage with incarcerated individuals pre-release to plan re-entry.
• Deliver post-release follow-up for up to six months.
• Coordinate services across behavioral health, housing, employment, and healthcare.
• Document services in accordance with program guidelines.
• Travel to multiple locations; some telehealth may be used.
