Job summary

Volunteers of America of Massachusetts seeks a Community Support Navigator in Behavioral Health Services. The full-time, hybrid role offers a starting salary of $55,000 per year and focuses on recovery support navigation for a high-need caseload across VOAMASS sites and community settings.

You will provide care management, system navigation, and wrap-around services in collaboration with a multidisciplinary team, with trainings provided on hire.

Qualifications

• Bachelor's degree in psychology, social work, health care administration, or related human service fields.
• Minimum of two years’ experience with social service organizations, case management coordination, and service connection.
• Excellent communication, organization, and computer skills; valid driver’s license and reliable transportation required.
• Desirable knowledge of MA resources and system navigation; training for Co-occurring Community support work provided on hire.

Responsibilities

• Engage with persons served after admission to help access treatment services.
• Meet clients in the community or at VOAMASS sites to identify needs, create service plans, and support action steps.
• Coordinate with referral providers to link clients to housing, primary care, mental health, and recovery supports.
• Provide follow-up to ensure attendance and engagement in treatment and goals.
• Maintain records per VOAMASS policies and ensure timely documentation.
