Volunteers of America Massachusetts is seeking a Community Support Navigator to provide community-based case management, care coordination, and recovery support for individuals facing mental health and housing challenges, homelessness, or justice involvement.

You will connect clients to behavioral health treatment, housing, employment, healthcare, and other essential resources, including specialized re-entry support for transitioning from Essex County correctional facilities.

Qualifications

• Bachelor’s degree preferred in psychology, social work, healthcare administration, or related field.
• Minimum of two years of experience with social service organizations.
• Experience with correctional facilities or re-entry services is preferred.
• Valid driver's license and reliable transportation required.
• Knowledge of system navigation and community-based resources in Massachusetts preferred.

Responsibilities

• Maintain a caseload of individuals enrolled in Community Support Programs, including those transitioning from correctional facilities.
• Provide intensive, individualized case management and care coordination.
• Engage with incarcerated individuals prior to release to develop re-entry plans.
• Offer post-release follow-up for up to six months.
• Conduct screenings and assessments using ORAS, CAAPE-5, and MSDP after training.
• Develop Wellness and re-entry plans addressing behavioral health, housing, employment, education, and healthcare.
• Coordinate access to services including housing, MAT, transportation, and public benefits.
• Collaborate with correctional facilities, courts, and community partners to support re-entry.
• Deliver flexible, mobile, community-based services, including telehealth when appropriate.
• Ensure documentation aligns with eligibility, billing, and grant guidelines.
• Travel to multiple locations and maintain timely service delivery.
• Perform related duties to support VOAMASS's integrated care model.
