Job summary

GEO Reentry Services LLC is hiring for a role focused on coordinating transition from in-prison programs to community reintegration. The position involves working with inmates, CDCR staff, and STOP programs to ensure seamless care and continuity of services.

Responsibilities include facilitating transportation, developing transition plans, and maintaining thorough documentation. A valid driver’s license and gate clearances are required.

Qualifications

• High school diploma or equivalent.
• Experience with case management and criminal justice populations is preferred.
• Must qualify for gate clearances into CDCR institutions.
• Must possess a valid driver's license.
• Must understand CBT, addiction theories, and related criminogenic factors.
• Knowledge of California Treatment Initiative and transition care continuum.
• Excellent English and spelling skills.

Responsibilities

• Provides interactive services to inmates and CDCR staff.
• Confers with the STOP Transportation Coordinator to facilitate participant transportation.
• Assists in development of community transition plans.
• Assists with appropriate referrals to community programming.
• Keeps the AOR apprised of the community transition plan.
• Conducts ongoing contacts with inmates and staff to develop community transition plans.
• Ensures all pre-release contacts are documented in the STOP case management file.
• Maintains documentation at the STOP Placement Office (sign-in sheets, attendees).
• Performs other duties as assigned.
