Job summary

GEO Reentry Services LLC is seeking a dedicated professional to coordinate with STOP Placement Office, In-Prison Program Transitional Counselor, AOR, and Community Transition Program staff to ensure continuity of care from in-prison programming to the STOP network of CBP programs.

Responsibilities include conducting ongoing inmate contacts, developing community transition plans, documenting pre-release activities, coordinating transportation, and presenting outreach programs to CDCR

Qualifications

  • High school diploma or equivalent.
  • Experience with case management and criminal justice populations preferred.
  • Must qualify for gate clearances into CDCR institutions.
  • Must possess a valid driver's license.
  • Knowledge of CBT, addiction theories and criminogenic risk factors.
  • Strong communication and organizational skills.
  • Ability to use standard computer software.

Responsibilities

  • Provides interactive services to inmates and CDCR staff.
  • Confers with STOP Transportation Coordinator to facilitate participant transportation.
  • Assists in development of community transition plans.
  • Assists with referrals to community programming.
  • Maintains documentation in STOP Placement Office files.

Responsibilities

Summary

This position is responsible for conducting ongoing contacts with inmates and coordinating with STOP Placement Office, In-Prison Program Transitional Counselor, AOR, and Community Transition Program (CTP) staff to ensure continuum of care from in-prison programming services to the STOP network of CBP programs.

Primary Duties and Responsibilities

  • Provides interactive services to inmates and CDCR staff.
  • Confers with the STOP Transportation Coordinator to facilitate the transportation of participants.
  • Assists in development of community transition plans.
  • Assists with appropriate referrals to community programming.
  • Keeps the AOR apprised of the community transition plan.
  • Conducts ongoing contacts with inmates, In-Prison Programs Transitional Counselor, STOP Placement Office staff, AOR, and CTP staff beginning one hundred fifty (150) calendar days prior to release in order to develop community transition plans.
  • Ensures all pre-release contacts are documented in the STOP case management file.
  • Conducts pre-release contacts to include:
    • Two (2) face-to-face contacts with inmates, CSR, and In-Prison
    • Programs Transitional Counselor. Two (2) contacts with the designated STOP Placement Office staff and CSR.
    • One (1) telephone contact with the AOR and CSR.
    • Responsible for Global Presentations to provide outreach presentation as follows:
    • Presents quarterly at all CDCR institutions within the Program Area, or locations deemed appropriate by CDCR.
    • Schedules the Global Presentation in coordination with the In-Prison Program Contractor.
  • Maintain documentation at the STOP Placement Office, including sign-in sheets that include the following:
    • Name of the Institution
    • Date and Time of Presentation
    • Name and Signature of the CSR Presenter
    • Name, CDCR number, and signature of each attendee
  • Performs other duties as assigned.
Qualifications

Minimum Requirements

  • High school diploma or equivalent.
  • Experience with case management and criminal justice populations is preferred.
  • Must qualify for gate clearances into CDCR institutions.
  • Must possess a valid driver’s license.
  • Must be able to understand a variety of models including cognitive behavioral therapy, theories of addiction and dynamics related to substance use and other criminogenic risk factors.
  • Knowledge in the California Treatment Initiative, in-custody and reentry treatment continuum of care, and the critical nature of portal-to-portal transportation in the retention process of Participants.
  • Extensive knowledge of the transitional process, both documentation and operational, between in custody and community based modalities/programs.
  • Possession of excellent English and spelling skills.
  • Possession of a level of maturity to deal with sensitive issues inherent in the delivery of reentry substance abuse treatment services to the criminal justice population.
  • Knowledge and the ability to comply with program documentation requirements, applicable ethical and professional standards of conduct.
  • Excellent communication skills.
  • Possession of excellent organizational and time management skills, as well as an ability to train subordinate office staff in use of same.
  • Ability to read and interpret documents such as government regulations, legal documents, operating instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Able to establish rapport with participants including management of crisis situation and determination of need for additional professional assistance.
  • Ability to provide effective oversight of participants.
  • Ability to facilitate the participant's engagement in the treatment and recovery process.
  • Possession of the case management skills necessary to implement the transportation plan, consult as a member of a multi-disciplinary team, and assess ongoing participant needs.
  • Ability to work with computers and the necessary software typically used by the department.

GEO Reentry Services offers correctional agencies in-custody treatment programs for individuals nearing the end of their jail or prison sentences. Starting treatment before they are released back into the community facilitates consistency in treatment and helps individuals be better prepared for their transition to the community. By assessing an individual’s risk and needs and initiating programming while in prison, the transition to a community-based program is seamless and behavior change is expedited.
