Qualifications

• Associates degree preferred. High school diploma or equivalent required; additional education or relevant experience in human services, nonprofit services, animal welfare, community outreach, case coordination, or a related field preferred.
• Minimum of one year experience with intake, case coordination, eligibility screening, resource navigation, community programs, customer service, animal welfare, or related work preferred.
• Ability to gather and evaluate information, identify needs, and consistently apply established eligibility and program guidelines.
• Strong organizational skills and attention to detail with the ability to manage multiple cases, requests, and priorities.
• Strong interpersonal and communication skills with a compassionate, respectful, and culturally responsive approach.
• Ability to maintain confidentiality and appropriate professional boundaries when working with sensitive information.
• Ability to recognize situations requiring escalation and appropriately involve the Community Support Manager when necessary.
• Ability to recognize patterns and clearly communicate needs, barriers, and observations to leadership.
• Comfortable participating in grassroots outreach and working in both shelter, clinic, and community settings.
• Proficiency with Microsoft Office and ability to learn organizational databases and tracking systems.
• Valid driver's license.

Responsibilities

• Program Intake & Case Coordination: Gathers information, screens requests, assesses needs, coordinates approved services, and maintains case documentation.
• Program Operations & Resource Coordination: Manages applications, forms, records, and logistics for community programs and resources.
• Grassroots Outreach & Community Engagement: Conducts outreach, participates in events, and shares program information with communities.
• Data Tracking & Reporting: Collects and maintains intake data, tracks participation and outcomes, and supports reporting and evaluation.
