Job summary

Universal Community Health Center (UCHC) is seeking a Patient Navigator to assist patients with navigating community resources, social services, and medical systems. The role acts as a liaison between UCHC and the community, coordinating assessments, care planning, and case management activities, including RPM program oversight.

You will collaborate with health educators, social workers, and medical staff, travel to patient homes, and provide bilingual English/Spanish communication to support

Qualifications

• High school diploma required; Bachelor’s degree preferred.
• 1 year of experience in community-level health education or related fields.
• Valid driver’s license and insurance; travel to patient homes and escort patients.
• Two years of experience working with vulnerable populations, homeless individuals/families, and those suffering from mental health disorders including substance use disorder.

Responsibilities

• Collaborate with health education participants onsite and in the community.
• Provide culturally sensitive health education and support diverse populations.
• Build and maintain relationships with community members and stakeholders.
• Coordinate multiple priorities and manage timeliness effectively.
• Coordinate activities related to patient centered care that involves identifying patient populations and providing one-on-one support to help them navigate through the healthcare system.
• Identify gaps in social determinants of health and document appropriately.
• Deliver patient education and assist with melanoma cancer prevention.
