Job summary

Open Sky Community Services Inc. seeks an Outreach Counselor to bridge clients with community resources and supports. You will deliver interventions under licensed supervision and help individuals pursue meaningful roles and relationships in the community.

Responsibilities include coaching clients, leading 1:1 and group activities, and guiding access to employment or entitlements while participating in on-call rotations. Emphasis on collaboration and person-centered care.

Qualifications

• High School Diploma, GED or equivalent required.
• Valid Driver's License and reliable, insured vehicle.
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
• Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
• Provide side-by-side support and coaching to help individuals socialize.
• Conduct both 1:1 and group activities geared toward increasing Persons' readiness to engage in rehabilitation.
• Ensure individuals have adequate financial support through assisting with finding gainful employment or applying for entitlements.
• Participate in the On-Call rotation.
