Job summary

Open Sky Community Services in Massachusetts is seeking an Outreach Counselor to connect Persons served with community resources, coordinate treatment, and support employment and education goals. You will deliver interventions per the Treatment Plan under licensed supervision, run individual and group activities, and help clients access entitlements.

Qualifications

• High School Diploma, GED or equivalent required.
• Valid Driver’s License and reliable, insured vehicle required.

Responsibilities

• Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
• Implement activities that develop valued roles and personal relationships in the community.
• Provide side-by-side support and coaching to help individuals socialize.
• Conduct 1:1 and group activities to increase readiness for rehabilitation.
• Assist with finding gainful employment or entitlements to ensure financial support.
• Participate in the On-Call rotation.
