Open Sky Community Services is seeking an Outreach Counselor in Massachusetts to coordinate treatment with community agencies and families, and to empower individuals served towards independence. You will collaborate with licensed staff to deliver interventions and support clients in 1:1 and group activities.

The role emphasizes employment support, community integration, and building readiness for rehabilitation, with a focus on trauma-informed care and inclusive work culture.

Qualificações

• High School Diploma or GED required.
• Valid Driver’s License, reliable and insured vehicle required.
• Reliability, willingness to learn, and openness to new opportunities.

Responsabilidades

• Deliver interventions identified on the Treatment Plan with supervision.
• Create opportunities for the development of valued roles and relationships in the community.
• Provide side-by-side support and coaching to help individuals socialize and engage in rehabilitation.
• Assist with obtaining gainful employment or entitlements to ensure financial support.
