Daybreak Independent Services Inc. is seeking an Intake Outreach Specialist in Bronx, NY. The role focuses on promoting agency programs, building relationships with OPWDD stakeholders, and guiding individuals and families through enrollment and referrals.

The candidate will collaborate with the Intake Specialist to ensure timely processing, maintain confidentiality, and represent the agency professionally during community outreach activities across Queens, Bronx, and Westchester.

Qualifications

• Bachelor's degree preferred.
• Minimum two years working with individuals with IDs/healthcare/human services.
• Knowledge of OPWDD services and eligibility preferred.
• Proficiency with Microsoft Office and electronic records systems.
• Driver's license and reliable transportation required.

Responsibilities

• Promote agency programs and services to referral sources.
• Develop relationships with OPWDD stakeholders and care coordination orgs.
• Conduct outreach visits, presentations, and informational meetings.
• Attend community events and networking opportunities to raise visibility.
• Respond promptly to referral inquiries and assist with enrollment process.
• Coordinate referrals with Intake Specialist to ensure timely processing.
