Catholic Charities Brooklyn and Queens, Inc. seeks a Community Outreach Specialist to collaborate with staff and volunteers in coordinating intake screening, needs assessments, information/referral, and follow-up. This role serves across Brooklyn and Queens in satellite locations and events.

Responsibilities include documentation, community linkages, and engagement in outreach activities that empower clients and strengthen the community; occasional evenings/weekends and travel are required.

Qualifikationen

• Bachelor’s degree in Social Work, Human Services, or related field.
• 2 years of experience with low income families preferred.
• Excellent written and oral communication skills.
• Ability to work flexible hours including evenings/weekends.
• Ability to travel across Brooklyn and Queens.

Aufgaben

• Travel to sites throughout Brooklyn and Queens to provide service coordination.
• Provide intake documentation, needs assessment, referrals, and follow-up.
• Collaborate with Mission Office to respond to emergencies.
• Assess eligibility for Emergency Food and Shelter and other funds.
• Develop internal/external linkages and promote program services.
• Document services and referrals thoroughly in the agency database.
• Track emergency funds usage in the agency database.
• Coordinate with Call Center for access to services.
• Participate in program planning, staff meetings, and reviews.
• Support on/off-site events with food pantries, parishes, and community sites.
