Job summary

FirstLight Home Care of Montclair is seeking an Intake & Outreach Specialist to guide local families through non-medical care transitions. The role involves field work across Chino, Montclair, Pomona, and Ontario, plus representing the agency at community events with warmth and empathy.

You will manage inquiries, conduct in-home assessments, and help build individualized care plans while expanding our local network.

Qualifications

• Experience in home care, healthcare coordination, social work, or related roles preferred.
• Must possess strong communication skills and empathy when engaging with families.
• Calibrated to work in California and navigate local community networks.

Responsibilities

• Handle inbound inquiries with warmth, assess client needs, and guide families through care options.
• Conduct in-home assessments and help create individualized service plans.
• Engage with community groups, senior centers, and local health events to raise awareness.
• Provide hand-off notes to back offices and coordinate with scheduling/care teams.
• Track opportunities and conversions in CRM systems and ensure accurate data transfer.
