CLCF is seeking an Intake/Special Projects Coordinator in Manhattan to serve as the primary contact for referrals and guide clients through enrollment in Health Home Care Management services. The role requires organizing intake, documenting eligibility, and coordinating with families, providers, and referral sources.

The ideal candidate will be detail-oriented, able to engage diverse families, and comfortable working across systems such as ePACES, MAPP, and Foothold Care Management.

Competenze

• Associate's degree is required.
• Bachelor's degree in Social Work, Human Services, Psychology, Public Health, or related field preferred.
• 1–2 years of experience in healthcare, social services, care management, intake coordination, customer service, or related field.
• Strong organizational, communication, and time-management skills.
• Proficiency in Microsoft Office and electronic documentation systems.
• Ability to maintain confidentiality and manage sensitive information.

• Referral Management: Receive and process referrals and maintain referral information.
• Family Outreach and Engagement: Contact families and explain services; identify needs and goals.
• Referral Source Coordination: Respond to sources and gather supporting documentation.
• Eligibility Determination: Review referrals for eligibility and confirm diagnoses meet program rules.
• Consent and Documentation Management: Obtain enrollment forms and explain processes to families.
• Enrollment and Assignment: Prepare intake files for enrollment and assign to care managers.
• Compliance and Quality Assurance: Ensure documentation is compliant and participate in audits.
• Special Projects: Support director with ad hoc initiatives and new workflows.
