Job summary

Exceptional Parents Unlimited, Inc. is seeking an Outreach Coordinator to participate in outreach events, connect with community partners, and educate the community about Help Me Grow services, early identification, and ASQ screenings.

You will work with the HMG Case Manager and Supervisor to promote services, maintain a caseload of 15-20 families in the county, and build collaborative partnerships while ensuring confidentiality and HIPAA compliance.

Qualifications

• Education: High School Diploma or equivalent; associate's degree preferred.
• 2+ years' experience with families and young children.
• Case management and knowledge of community resources preferred.
• Must work with diverse populations and be a team player.
• Bi-lingual in Spanish is a plus.

Responsibilities

• Assist in linking families with services and resources.
• Provide presentations about Help Me Grow and lead outreach events.
• Maintain own work assignments with minimal supervision.
• Engage with diverse communities respectfully and inclusively.
• Manage a caseload of 15-20 families in the county.
• Coordinate monthly Help Me Grow Coalition Meetings and take minutes.
