Job summary

Open Sky Community Services seeks an Outreach Counselor to coordinate treatment with individuals served, families, and community partners. The role emphasizes compassionate, evidence-based care and helping clients gain independent living skills.

You will support clients in employment pursuits, education access, and social rehabilitation while collaborating with multidisciplinary teams in a trauma-informed, inclusive workplace recognized as a Top Workplace.

Qualifications

• High School Diploma, GED or equivalent (required).
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
• Implement activities to develop valued roles and personal relationships in the community.
• Provide side-by-side support and coaching to help individuals socialize and engage in rehabilitation.
• Help individuals secure financial support by finding employment or applying for entitlements.
• Participate in the On-Call rotation.
