Job summary

Open Sky Community Services in Massachusetts is seeking Outreach Counselors to join a multidisciplinary team delivering evidenced-based interventions and assisting adults with mental illness or dual diagnoses in achieving independent living. You will coordinate treatment and support employment and education initiatives while fostering community connections.

You will provide direct 1:1 and group activities, help clients develop social skills, and participate in on-call rotations to ensure

Qualifications

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License with reliable, insured vehicle and acceptable driving record.
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
• Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
• Provide side-by-side support and coaching to help individuals socialize. Conduct both 1:1 and group activities geared toward increasing Persons’ readiness to engage in rehabilitation.
• Ensure individuals have adequate financial support through assisting with finding gainful employment or applying for entitlements.
• Participate in the On-Call rotation.
