Job Description

The Always Best Care Community Outreach Coordinator is responsible for managing community outreach and business development in assigned territories. The role involves meeting or exceeding marketing and outreach objectives, making daily decisions related to outreach to potential clients, territory management, customer service, and implementing outreach/marketing plans. The position reports directly to the Director of Marketing and Community Outreach.

Essential Duties and Responsibilities
  • Provide information and education to healthcare professionals, facilities, organizations, and consumers about services offered by the company.
  • Maintain in-depth knowledge of the community, healthcare facilities, and referral sources relevant to our Home Care Services.
  • Develop and maintain relationships with discharge planners, case managers, social workers, physicians, and other healthcare professionals through regular visits to assist in identifying appropriate clients for services.
  • Track key performance indicators (projected vs. actual) using company metrics and ad hoc reports.
  • Collaborate with team members and managers to develop outreach strategies.
  • Follow-up on all leads sourced from Lead Generator.
  • Make routine visits to established, pending clients and conduct cold calls (current referral list will be provided).
  • Mentor and train new Community Outreach staff.
  • Prepare statistics, trends, and updates for routine staff meetings and as needed.
  • Coordinate new client intakes with customer service to ensure timely initiation of care.
  • Represent Team Greene- ABC at networking meetings, community events, and in-services.
  • Maintain professionalism at promotional events.
  • Participate in budget discussions regarding expenditures, advertising, and new initiatives.
  • Assist with marketing logistics like event production, booth scheduling, collateral coordination, and notifications.
  • Report activities regularly to the Director of Marketing and Community Outreach and discuss strategies for new business.
  • Measure outreach effectiveness and produce impact reports.
  • Perform other duties as assigned.
Work Environment

This role operates in a professional office setting, utilizing standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.

Position Details

This is a full-time position, Monday through Friday, with potential evening, weekend, and holiday hours for events and community activities. Up to 75% travel is required, necessitating a valid driver’s license, reliable transportation, and insurance meeting state guidelines.

Qualifications
  • Bachelor's degree in Business, Communications, Healthcare Administration, or Social Services preferred; or 1-5 years experience in B2B or healthcare environments; or 1-3 years healthcare marketing/outreach experience, preferably in home health or hospice.
  • Bilingual skills are a plus.

  • Proficient in email, internet, Microsoft Word, Excel, and PowerPoint.
  • Detail-oriented with a commitment to quality work.
