Job summary

UNC Health in North Carolina is seeking a Community Health Outreach Coordinator to support CHE department outreach efforts. The role advances access to care through language access support, patient navigation, and health education at community clinics and events.

The position collaborates with leadership to promote accessible health services, provides interpretation as needed, and helps maintain program data and narratives. Hybrid work with some evenings/weekends required.

Qualifications

• Bachelor's degree from four-year college or university preferred in a related field.
• Proficient with Microsoft Office suite and data/document handling.
• Excellent oral and written communication, public speaking.
• Spanish bilingual preferred.

Responsibilities

• Supports community outreach events, including health clinics and forums; plans, coordinates and executes events.
• Supports CHE Mobile Health clinics and telehealth visits; provides interpretation as needed and documents in EPIC EHR.
• Provides navigation to connect patients to health services and social determinants of health resources.
• Keeps current on health trends and UNC Health programs via workshops and community events.
• Educates community members on health topics and distributes evidence-based materials.
• Translates documents and interprets to increase accessibility for non-English speakers as needed.
• Maintains program data; develops narrative reports, presentations, tables and graphs.
• Performs other duties as assigned.
